You just have to keep trying. Focus is very important, don't allow your mind to drift. It'll take a lot of perseverance, but you're on the right track. Good luck :)

Varies for different people, and of course how sleepy you are etc. Expect something like 15-30 minutes.

To re-visit an old dream, I'm afraid that requires a bit of experience. The easiest way would be to induce a WILD, and therefore concentrate on that dream as you fall asleep, hopefully initiating the same scene. A more tricky way would be to become lucid midway through a dream by doing reality check...
